The annotated form does an excellent job of explaining the details of filling out the form – study it.

Consistency is vital. Ensure that publisher names are consistent with the way we’ve listed them already on the website. Also be careful to ensure that your own reviews are consistent with each other. If you review several items from the same publisher, be consistent in the manner in which you list both publisher name and supplier information. I cannot tell you how many times I’ve received multiple reviews from one reviewer, all dealing with the same publisher and/or supplier, that had differing information, company names presented differently, and were inconsistent in other ways.
